Indications:

Sulcus Sign test is a shoulder stability examination to determine if there is anterior or multidirectional instability observed between the acromion and the humeral head.

Procedure:

  1. Patient seated or standing with arm hanging at side
  2. Examiner observes from anterior

Interpretation:

Positive Abnormal prominence of acromion & groove like depression below acromion → Inferior instability, glenohumeral dislocation, atrophy of the deltoid muscle.

Clinical notes:

Graded by measuring from inferior margin of acromion of humeral head

I 1< cm inferior translation
II 1-2 cm inferior translation
III >2 cm inferior translation
