Modified Helfet’s Test

Procedure

  1. Patient seated with knees flexed 90°
  2. Examiner notes the location of the tibial tuberosity in relation to midline patella
  3. Examiner then passively extends the patient’s knee and again notes relationship of tibial tuberosity to midline patella

Interpretation

Positive Modified Helfet’s Test

Tibial tuberosity remaining in line with the patella during extension: meniscal lesion or tear (blocked tibial rotation)

Clinical Notes

Normal: when the knee is flexed 90° tibial tuberosity is midline patella, with extension of the knee the tibial tuberosity moves in line with the lateral border of the patella.
